SCOPE OF THIS CHAPTER
The aim of this checklist is to assist foster carers, Personal Advisers, Independent Reviewing Officers and young people to identify tasks and issues that need to be addressed in the development of a Pathway Plan.
Leaving Care Planning Checklist:
